By signing below, I understand and will agree to the

You agree to have this pet examined by a vet of your choice within 72
hours for a well check and will maintain regular vet care as prescribed by your
vet. You further understand and
agree that Bichon Rescue shall not be responsible for any veterinarian expenses
associated with said examination or any resulting treatment.

Should this pet be under 6 months old, and un-neutered, you agree to have
this pet neutered/spayed at 6 months of age, or sooner, if your vet agrees it is
safe to do so.

You understand that a Bichon is a grooming type pet, and that regular
grooming is essential to his health. To
neglect regular grooming will cause health problems for the pet, for his coat,
his ears, and can lead to allergies, skin problems, and disease.
* If
this pet shows any sign of aggressive behavior, or bites anyone, you will notify
BICHON RESCUE IMMEDIATELY!

You understand adoption fees are charged to help defray the cost of the
rescue work provided by Bichon Rescue and may or may not reflect actual costs
incurred on this particular rescue. Your
adoption fee of $__________ will go towards the care and placement
of other rescue animals, and is
not refundable.
* Should you find you are unable to care for this
rescue, you will return him to Bichon Rescue! Under no circumstances will this
pet be taken to an animal shelter.
* You
acknowledge receipt of this rescue, and understand there are NO warranties
(expressed or implied) given relative to this animal by Bichon Rescue, and that
information relative to the animal’s previous ownership, AKC Status,
health condition or otherwise, may not be available to Bichon Rescue, and
AKC papers if available, will be returned to AKC to void the registration.

You have been given as much information as was available to Bichon Rescue
for the placement of this pet, and you are agreeable to adopt this pet as is,
for LIFE, promising to care for him as you would any member of your family,
through sickness and health, till death do you part.
